About Home Modifications and therapy services
Our Goal
Our goal at Home Modifications and Therapy Services LLC is to help clients and their families maintain their independence and quality of life at home by providing home safety assessments, fall prevention trainings, in-home occupational therapy services, adaptive equipment recommendations and caregiver trainings.
Currently we are able to provide in person services in Charlotte (and surrounding areas), Waxhaw, Matthews, Indian Trail, Gastonia, Marvin, Weddington, Lancaster, Indian Land, Fort Mill, and Rock Hill and are available for video telehealth consultations in other locations in both states.

Thank you for visiting my page; this is a great first step to being able to stay at home. My name is Matt Schwartz, and I am a licensed Occupational Therapist. I have worked in a variety of settings and found my niche in Inpatient Rehab, providing therapy to patients with various conditions with the goal to be safe and independent prior to discharge. I am now looking forward to expanding my passion so I can actually see patients in their home environment to offer solutions and advise in real time to decrease risk of falls, increase safety awareness and improve their quality of life.
As an occupational therapist and a Certified Living in Place Professional (CLIPP), I am skilled in taking a holistic approach to assess and treat the physical, mental, developmental, and emotional ailments that impact a client's ability to perform everyday tasks.

About Home Mods
Options for Payment
Most of the time, we only need a few sessions to evaluate the situation and come up with and implement a solution oriented action plan. We are an out of network provider and will give you an itemized bill to submit to your insurance company including Medicare for potential partial to full reimbursement. For those who have a Flexible Spending Account or Health Savings Account, the good news is that Occupational Therapy is considered a covered service as per IRS Pub 502 and should fully cover our visits once you submit a paid bill.
